Each person on this roster agrees to the following disclaimer. Submitting this form is equivalent to signing your names to the following.

For and in consideration for my participation in the Buffalo Social Club, Inc., 2007 Volleyball League (hereinafter the League), I hereby, for myself, my heirs, executors, successors and administrators assume any and all risks associated with my participation in the league, and release and waive any and all rights and claims that I may now, or in the future, have against the Buffalo Social Club Inc., its agents, employees, directors, officers and affiliates of the league, arising out of my participation in the league or any related activities. I hereby fully release and discharge the Buffalo Social Club Inc. and its agents, employees, directors, officers and affiliates from any and all claims from injuries, damage or loss, including, but not limited o, any alleged negligence, which I may have or which may accrue to me from my participation in the league. I further agree to indemnify and hold harmless and defend the Buffalo Social Club, Inc., its agents, employees, directors, officers, and affiliates from any and all claims resulting from injuries, damages and losses sustained by me and arising out of, connected with or in any way associated with the activities of the league. I acknowledge that the Buffalo Social Club, Inc. is not responsible fit any inclement weather which may affect the Leagues and release and waive all claims against the Buffalo Social Club, Inc., its agents, employees, directors, officers ad affiliates for damage of loss that may arise there from.
